Immunization Clinic

To the Parents/Guardians of 6th and 12th Grade Students,

All students entering 7th grade in the state of Texas are required to have TDaP (Tetanus/Diphtheria/Pertussis) and

MCV (Meningitis) vaccines upon entry into 7th grade.

Students entering college are also required to have a meningitis booster (MCV) 

Liberty Eylau ISD is partnering with Aurora Concepts to offer these vaccines to our students.  On February 12, nurses from Aurora Concepts will be on the middle school and high school campuses to administer the immunizations to those that sign up. 

*All insurance policies will be verified, please sign up with the link if you plan to participate!

*If you plan to pay out of pocket, please submit your info!  You will put “NA” in all insurance blanks.

$5/shot – up to 18 yo

Sign up is quick and easy.  You may sign up online at:

Or by picking up a form from the school nurse and returning it with a copy of your insurance card.  This service makes it easy for students to receive the necessary vaccine without missing school.  Each student that signs up will be called from class to have the vaccine administered and will be allowed to return as soon as it is done.

 **DEADLINE TO SIGN UP/RETURN CONSENT FORM IS FEBRUARY 5, 2021**

Please take advantage of this opportunity as vaccine supplies are usually limited in the fall when the demand is high.
